I hereby certify as of Friday, July 29, 2005, I will be 21 years of age or older. I agree not to hold Trident Int. Windy City NFP, it’s members, trustees, officers, contractors, guests, hotels or owners of event locations used as part of Neptune II; responsible for any accident, injury, harm or claim caused to my person or any loss or damage to my property.

I assume full responsibility for any and all damage I may cause to the property of Trident Int. Windy City NFP, any event location, The Inn at Lincoln Park, any guest or vendor property. Further I agree to wear appropriate covering in all public places. I also agree not to bring any pets or substances that are illegal or not prescribed to me by a physician. I understand that I must make my own reservations with the hotel using the information provided hereon. I further understand that refunds for accommodations are subject to the hotel’s own policy and not under control of Trident Int. Windy City NFP.
